I haven't done deadlifts in a while, and I kind of feel like my form might be a little off. https://youtu.be/5LxVAV4yvRE

I don't want to put any real weight on the bar until I have everything down so I don't hurt myself again.

For the most part your form looks good.

1. Get flat shoes or do this barefoot. Those shoes are making you push more off your toes.
2. Stop COMPLETELY at the bottom before going into next rep. do a DEAD STOP.
3. Looks like you are still doing a bit of a 1... 2.. motion where your butt goes up first, then you pull the bar. Needs to be one smooth motion.. are you doing deficit deadlifts? they will help you get better at pushing with glutes and hamstrings. check this article out: http://muscularstrength.com/article/Deadlift-Why-You-Are-Not-Lifting-As-Much-Weight-As-You-Could
