For the most part your form looks good.
1. Get flat shoes or do this barefoot. Those shoes are making you push more off your toes.
2. Stop COMPLETELY at the bottom before going into next rep. do a DEAD STOP.
3. Looks like you are still doing a bit of a 1... 2.. motion where your butt goes up first, then you pull the bar.
